  • Accident Investigation Forum

    I am getting ready to update our accident investigation form.  I looked at the Pennsylvania Labor & Industry form on BLR.  Does anyone have other suggestions?

    I would also appreciate any examples.  One thing we want to add is a space for "incidents"--something like a bee sting (no allergic reaction), or a small vehicle bump (no damage) in order to streamline the reporting & review process but still have documentation.

    bcorridon - can you share with my your accident investigation form? I am in the midst of creating our accident investigation program for our company.  I went to the training sponsored by our chamber offices and received everything but the accident investigation form.

    Speak with your workmans comp administror, the incident investigation form is a very important document to just throw together.

    I found that the Chesapeake Region Safety Council has been very helpful in providing many resources in accident investigation and prevention programs. Since the Western Pa. Safety Council folded several years ago, I found this chapter and they have been superb in their offerings.

    Forms are great, but only if they are linked to something that will allow you to gather historical data. In other words, a paper form is good for documenting the incident, but then you file it away and that's it. Consider taking your document and recreat it in MS Access. Now you  can enter the data and at a click of the mouse generate reports for any type of information that might be needed. So, regardless of the type of form that you end up with, make it useful. Access is a great tool for that.

    I do fire investigations and safety training, I think the db is best. Any way you go remember you are on a fact finding mission and not a fault finding mission.
