  • Form 300 and 300A Completion

    If a worker suffers an injury that requires "Days away from work" then comes back with restrictions, do you have to check both boxes on the Form 300 ("Days away from work" & "Job transfer or restriction")? I am under the impression that you only check the most severe category, so in this case you would only check the "Days away from work" block.

    If he was away from work for 30 days and then restricted for another 30 days, do you enter "30" under each column ("Away from work" & "On job transfer or restriction") or do you enter 60 in one or the other column?

    Thanks for any help you provide.

    Your impression is correct.  As the OSHA 300 log states above columns G,H,I and J "Check only one box for each case based on the most serious outcome for that case."  Also, yes to the second part.  You would put the number of days for each item in the appropriate columns.

    1904.32 has to do with the annual summary.  This is taken from your OSHA 300 log.  So document it as discussed and the 300A (annual summary) will take care of its self.
